⚠️ Important Distinction: Online Profile vs. Medical Records
Before submitting a request, please understand the difference between your online user account and your permanent medical records:
1. Your Online User Profile (Can be Deleted)
This includes your login credentials for Great Smile Online, your saved preferences, profile photo, and website usage history.
- Action: Upon request, we can delete your online account. You will no longer be able to log in, view past appointments online, or receive automated SMS/Email reminders from the portal.
2. Your Medical/Dental Records (Cannot be Deleted Immediately)
This includes your clinical charts, X-rays, treatment history, insurance claims, and diagnosis notes.
- Legal Requirement: As a healthcare provider in the State of Illinois and under federal HIPAA regulations, we are required by law to retain patient medical records for a specific period (typically 10 years for adults) for legal and continuity-of-care purposes.
- Action: We cannot "delete" these records upon request. They will be securely archived and eventually destroyed only after the legal retention period has expired.
What Happens When You Submit a Request?
- Verification: To protect your health information, we must verify your identity before processing any data removal request.
- Account Deactivation: Your access to the Great Smile Online booking platform will be terminated.
- Marketing Removal: Your email and phone number will be removed from our marketing lists.
- Archiving: Your medical health records will be moved to a secure "Inactive" status in our internal practice management software, accessible only by authorized clinical staff as required by law.

To initiate a data deletion request, please fill out the form below or contact our Privacy Officer directly.
By Email: Please send an email to office@greatsmiledeerfield.com with the subject line: "Data Deletion Request - [Your Full Name]".
Required Information:
- Full Name
- Date of Birth
- Phone Number associated with the account
- Reason for request (Optional)
Processing Time: Please allow up to 30 days for us to process your request and confirm the deletion of your online profile.
